

Starting an Intensive Outpatient Program (IOP) can feel overwhelming, but at BOLD Health in San Diego, we’re here to support you every step of the way. Whether you’re beginning treatment for mental health concerns, substance use, or both, our goal is to create a safe and welcoming space where you can heal and grow. If you’re feeling a little nervous about your first session, that’s completely normal. Here’s a step-by-step breakdown of what you can expect.

Before Your First IOP Session
Before attending your first IOP group, you will have already completed two important components of the intake process:
- Clinical Assessment: This initial evaluation helps us understand your needs, goals, and history so we can customize your treatment plan.
- Medication Management Appointment: If appropriate, you will also meet separately with one of our medical providers for a comprehensive psychiatric evaluation. This session is distinct from your therapy intake and ensures that your medication needs are thoroughly addressed as part of your care.
After your intake sessions are complete, you’ll receive a treatment schedule outlining your IOP sessions, which typically include group therapy, individual therapy, and ongoing psychiatric support.

Step 1: Arriving at BOLD Health
When you arrive at our San Diego clinic for your first IOP session, you’ll be warmly welcomed by our staff. Our facility is designed to offer a calm, comfortable environment that supports your healing process.
Note: All IOP sessions at BOLD Health are conducted in person. We do not offer virtual IOP services.
Step 2: Orientation and Introduction
Your first session will begin with a brief orientation led by one of our clinicians. They’ll walk you through the structure of the IOP, what to expect in group therapy, and how to make the most of your experience. You’ll also be introduced to our group guidelines around confidentiality, mutual respect, and active participation—key elements that help foster a safe and supportive space.

Step 3: Meeting Your Group
A valuable part of IOP is the opportunity to connect with others who are going through similar experiences. During your first session, you’ll meet fellow group members and have the chance to introduce yourself (only if you’re comfortable). Many participants are nervous about sharing in a group setting at first—this is completely normal. Just know that everyone is here for healing, and group members are often some of the most empathetic, supportive people you’ll meet.
Step 4: Engaging in Group Therapy
Group therapy is the heart of the IOP experience. In your first session, the therapist will lead a discussion or activity focused on therapeutic themes. You are welcome to participate as much or as little as you feel ready for. Topics often include:
- Coping skills for managing stress and emotional triggers
- Identifying and reframing negative thought patterns
- Building healthy relationships and communication tools
- Setting and working toward personal goals
If you’d rather observe during your first session, that’s completely okay. Many participants find value in listening and easing into group discussions over time.
Step 5: One-on-One Support
In addition to group sessions, IOP includes individual therapy to ensure you receive personalized attention. Your first IOP day may also include a brief check-in with your assigned therapist to begin building your one-on-one therapeutic relationship and clarify your personal goals.
